January is cervical cancer awareness month. We would like to tell you about cancer rehab for cervical cancer in the form of physiotherapy, exercise, and Pilates. Exercise has a significant role in the prevention of cancer, treatment of cancer, and recovery after treatment.
Cervical cancer
Cervical cancer is when abnormal cells grow in the lining of the opening to the womb from the vagina, this is the cervix. It is one of the cancers that affects the female reproductive system. In the UK there are around 8 people diagnosed with cervical cancer every day, it is the 14th most common cancer in women in the UK. It is more common in younger women and more than half the diagnoses each year are in women under the age of 45.
Nearly all cases of cervical cancer are caused by high-risk Human Papilloma Virus (HPV). In the UK all girls age 12 or 13 are routinely offered the HPV vaccine which protects against the types of HPV most likely to cause cervical cancer. These offer excellent prevention of cervical cancer but do not protect 100% so the cervical smear test used for screening the cells of the cervix is also important.
Cervical screening is offered on the NHS to all women aged 25 to 64. Cervical screening picks up any changes in the cells of the cervix that could potentially develop into cancer if left untreated. It is a vital part of preventing cervical cancer. You should be offered a cervical screening (smear test) every 3-5 years. Cells are collected from your cervix and sent to the laboratory to be tested for HPV and cell changes. This is done by opening your vagina using a speculum which is plastic or metal so that the nurse can insert a soft brush to take cells from the cervix at the top of your vagina. It is usually painless but can feel uncomfortable. If you have any concerns about having your smear test please do speak to your GP, it is vital to check for changes as early treatment is the most effective treatment!
Exercise to prevent cancer
Exercise has been shown as beneficial in helping to prevent many different cancers. In cervical cancer research has shown that even just 30 minutes per week of exercise has the potential to significantly reduce a woman’s risk of developing cervical cancer.
If you take part in physical activity or exercise more than 4 times per month you are significantly less likely to develop cervical cancer. This is still true when considering other risk factors such as smoking, alcohol intake and family history of cancer.
Put differently, women who do not take part in regular physical activity are 2.5 times more likely to develop cervical cancer.
If you are diagnosed with cervical cancer, taking part in regular physical activity reduces your risk of dying of the disease by 37%.
These numbers show that there is a powerful message that even small amounts of exercise such as 30 minutes per week can reduce your risk of cervical cancer. If you link that in with smoking cessation and reducing alcohol intake you will make an even bigger impact. These things have been shown to go together and support each other so if you seek support for increasing exercise you are more likely to stop smoking and reduce alcohol as well.
Cervical cancer treatment
Treatment for cervical cancer is individual to each patient depending on the type, stage and spread of the disease. Treatment can include surgery, which may be just the cervix or can include removal of other reproductive organs such as the uterus (hysterectomy) or to further organs in the pelvis depending on the spread of the disease.
Treatment can also include chemotherapy and radiotherapy which can be both pre and post-operative. The side effects of these treatments and of the surgery can significantly affect quality of life. Exercise and cancer rehab can have a profound positive effect on patient’s quality of life by managing these side effects of treatment.

When you are diagnosed with cancer it is hard to see past the initial surgery or treatment. The focus is often on the surgery, removal of the cancer, and the curative effects of chemo or radio therapy. This can sometimes mean that after that has happened and you are clear of the cancer you don’t necessarily have the guidance or support you may need to return to your normal life.

New research indicates that a single dose of the human papillomavirus (HPV) vaccine is as effective as multiple doses for preventing preinvasive cervical disease, which can later develop into cervical cancer. The findings are published early online in CANCER, a peer-reviewed journal of the American Cancer Society (ACS).
HPV is the most common sexually transmitted infection in the United States, and persistent infection with certain types of the virus can cause cervical cancer. To prevent infection, the Centers for Disease Control and Prevention recommends that adolescents — both boys and girls — under the age of 15 years receive a two-dose schedule of the HPV vaccine. To determine the effectiveness of other dose schedules, Ana M. Rodriguez, MD, MPH, of The University of Texas Medical Branch at Galveston, and her colleagues examined information on females aged 9 to 26 years who were unvaccinated or who received one or more HPV vaccine doses between January 2006 and June 2015.
The analysis included 133,082 females (66,541 vaccinated and 66,541 unvaccinated). For females ages 15 to 19 years, those who received one, two, or three doses of the HPV vaccine had lower rates of preinvasive cervical disease than adolescents who were unvaccinated. Within five years, 2.65 percent of unvaccinated teens aged 15 to 19 years developed preinvasive cervical disease, compared with 1.62 percent, 1.99 percent, and 1.86 percent in the one-, two- and three-dose groups, respectively. The risk of preinvasive cervical disease was 36 percent, 28 percent, and 34 percent lower for adolescents who received one, two, and three doses, respectively, compared with adolescents who were unvaccinated.
For the youngest (less than 15 years old) and oldest age groups (20 years and older), the investigators did not find significant differences among the vaccinated groups in terms of risk for preinvasive cervical disease.
“This study shows the impact of vaccinating at younger ages and its lasting long-term protection against cervical cancer,” said Dr. Rodriguez. “It is important to educate parents about the need to vaccinate their children.”
An accompanying editorial discusses the public health implications of the study’s findings. “If one dose of HPV vaccine was sufficient for effective protection, HPV vaccine implementation and scale-up would require less logistics…, available doses could extend further, and the overall cost would be lower,” the authors wrote.

Cervical cancer could be eliminated worldwide as a public health issue within the next century. This is the conclusion of two studies published today in The Lancet by an international consortium of researchers codirected by Professor Marc Brisson from Université Laval’s Faculty of Medicine and the CHU de Québec-Université Laval Research Centre. The researchers are even more optimistic about North America, stating that the disease could be almost completely eliminated by 2040.
The study used the human papillomavirus (HPV) vaccination and cervical screening targets defined in the WHO draft strategy of cervical cancer elimination. The plan calls for 90% of girls to be vaccinated against HPV by 2030. The plan also calls for 70% of women to be screened for cervical cancer once or twice in their lifetime, and for 90% of women with precancerous lesions or cervical cancer to receive appropriate treatment.
The researchers’ analyses show that with vaccination alone, the cervical cancer cases will drop by 89% within a century in the 78 countries worst affected by the disease, with 60 million cases of cancer averted as a result. By adding the two screening tests and the treatment of precancerous cervical lesions, cervical cancer cases will drop by 97% and 72 million cervical cancer cases will be averted over the next century. Furthermore, with scale-up of appropriate cancer treatment, 62 million cervical cancer deaths will be averted.
“For the first time, we’ve estimated how many cases of cervical cancer could be averted if WHO’s strategy is rolled out and when elimination might occur, says Marc Brisson. Our results suggest that to eliminate cervical cancer it will be necessary to achieve both high vaccination coverage and a high uptake of screening and treatment, especially in countries with the highest burden of the disease.”
The results were used to develop WHO’s cervical cancer elimination strategy which will be presented for adoption at the World Health Assembly in May 2020. “If the strategy is adopted and applied by member states, cervical cancer could be eliminated in high income countries by 2040 and across the globe within the next century, which would be a phenomenal victory for women’s health,” says Professor Brisson. “However, this can only be achieved with considerable international financial and political commitment, in order to scale up prevention and treatment.” “
